1. Shoulder flexion exercise  

  • Attach your pulleys over a door and stand with your back to the door.  
  • Hold the handle in both hands and pull the handle on your unaffected side down. 
  • The other hand will slowly rise. 
  • Allow your arm and shoulder to move forward and up while keeping your elbow straight. 
  • Lift your arm as far as possible.  
  • Hold your arm in the raised position for 2 seconds, then slowly bring it down to your side. 
  • Repeat this exercise 10 times. 

2. Shoulder abduction exercise  

  • Stand with your back to the door.  
  • Hold both handles of the shoulder pulleys and pull the handle down in your hand on the non-affected side.  
  • While doing this, keep your elbow straight and allow the pulleys to raise your injured arm to your side. 
  • Lift up until you feel gentle tension in your shoulder, and then hold the position for 2 seconds. 
  • Then using your non-painful hand and arm, slowly lower your painful arm and shoulder down to your side. 
  • Repeat this exercise 10 times. 

3. Should internal rotation exercise  

  • Stand with your back to the door and hold both handles of the exercise pulley 
  • Rotate your painful shoulder slightly inward while resting your hand comfortably behind your hip or low back. 
  • Use your non-painful side to pull the pulley handle down.  
  • While doing this, the hand behind your back will be pulled up by the pulley. 
  • Allow the pulley to pull your hand up your back until a gentle stretch is felt. 
  • Hold this position for two seconds. 
  • Then slowly bring the arm behind your back to the area just behind your hip. 
  •  Repeat this exercise 10 times. 

Note: You should work with your medical provider and physical therapist to ensure that the exercises you perform on the hand pulley are appropriate for your specific condition. Be sure to follow your physical therapist’s instructions closely. Aggressive exercises too soon after surgery can aggravate your condition.
