How to use Vasyli Red High Density Insoles?
Foot Orthoses Fitting Procedure
The primary concern when fitting the orthoses is to ensure that they are comfortable. This is the single most important factor in maximizing participant compliance with wearing the orthoses. Secondly, but nevertheless still importantly, you should aim to achieve a reduction in knee pain during aggravating activities.
Stage 1 - Examine footwear:
- Is it in good condition?
- Do the shoes need replacing i.e. is there fissuring or breakdown of the sole or midsole?
- Will the shoe be deep enough for the orthoses?
- Is the existing insole easy to remove?
- Is the heel cup firm and vertical?
- Is the last semi-curved i.e. not too straight or curved?
Stage 2 - Size the orthoses:
- Look at the shoe size and match it to the foot orthoses box
- Have the patient stand on the orthoses and check that the leading edge of the arch of the orthosis is approximately 5-10mm proximal to the 1st MTP joint (must allow unrestricted 1st MTP joint function)
- Remove the existing shoe insole
- Insert the orthosis into the shoe
- Make sure that it fits snugly in the heel cup and does not move around excessively
- If the leading (distal) edge is uncomfortable, you can use the existing shoe insole to improve this (with permission from the participant)
- Line up the 3/4 length orthosis with the existing shoe insole, and cut off the distal end of the insole at the level of the distal edge of the 3/4 length orthosis
- Put the distal end of the insole into the toe of the shoe, and fit the 3/4 length orthosis in behind it
- This reduces the drop of the leading edge
- Issue velcro buttons (in practitioner pack) and place on the bottom of heel of the orthoses and the heel region of the shoe
Stage 3 - Fit the orthoses:
- Make sure that the participant is wearing socks
- Put the orthoses into the shoes and ask the participant to put their shoes on
- Ensure that the orthosis is pulled right back to the heel cup and is flush against the medial inner lining of the shoe
- Use your thumb to hold the 3/4 length orthosis in place as the participant puts their foot in
- Reassess walking / aggravating activity
How to do Heat Molding?
- Take the orthosis out of one shoe, leaving the other shoe on
- Hold the heat gun 10-15cm away from the orthosis, and heat diagonally across the plantar surface of the orthoses just distal to the heel cup (over the writing, avoid the blue wedges), until the writing starts to disappear
- Put the orthosis back in the shoe and have the participant put the shoe back on (laced up)
- Find subtalar joint neutral by placing your finger and thumb on either side of the talus and your other hand on the distal tibia
- Guiding from the tibia, have the patient roll back and forth into pronation and supination until you find the point where the talus protrudes evenly on both sides; this is approximately subtalar joint neutral
- Ask the participant to stand with equal weight on both feet, looking straight ahead
- Maintain subtalar joint neutral by keeping your hands in position; hold this for about one minute
- Ask the participant to walk approximately 30 meters (e.g. two lengths of a practice hallway or carpark)
- Leave the orthosis in the shoe and repeat the above steps on the opposite side
